Consolidated Waiver

The Pennsylvania Consolidated Waiver is designed to help individuals with an intellectual disability, autism or developmental disability to live more independently in their homes and communities and to provide a variety of services that promote community living, including self-directed service models and traditional, agency-based service models.

Eligibility Criteria

The following diagnosis apply:

Mild Intellectual Disability

Moderate Intellectual Disability

Severe Intellectual Disability.

Developmental Delay

Developmental Academic Disorder

Autism Spectrum Disorder

ADHD

Anxiety Disorder

The following age requirements apply:

Adults over the age of 18.

Level of Care

Medical Evaluation

Diagnosis of an intellectual disability, autism, or developmental disability

Recommended for an intermediate care facility (ICF) level of care based on a medical evaluation.

Determined eligible for Medical Assistance (MA) in Pennsylvania

Meet the financial requirements as determined by your local County Assistance Office

Services we offer

Companion

Companionship care is available and offers company and friendship. Companions may help with certain household chores; however, their primary purpose is to spend time with you or your loved one.

In-Home and Community Support

Personal care is available for any daily tasks you may need support with, such as bathing, showering, applying creams and lotions, dressing and toileting, all in the comfort of your own home.

Respite

Respite care is available to provide temporary relief for a primary caregiver, enabling you to take a much-needed break from the demands of caregiving a sick, aging, or disabled family member.
