Community Living Waiver

Are you a friend or family member of an adult who requires service? You are eligible for weekly payments and a referral/sign-on bonus!

The Community Living Waiver supports individuals with an intellectual disability, autism or developmental disability to live more independently in their homes and communities through the provision of a variety of services that promote community living, employment, communication, self-direction, choice and control.

Eligibility Criteria

The following diagnosis apply:

Mild Intellectual Disability

Moderate Intellectual Disability

Severe Intellectual Disability.

Developmental Delay

Developmental Academic Disorder

Autism Spectrum Disorder

ADHD

Anxiety Disorder

The following diagnosis and age requirements apply:

Adults over the age of 18.

Level of Care

Medical Evaluation

Diagnosis of an intellectual disability, autism, or developmental disability

Recommended for an intermediate care facility (ICF) level of care based on a medical evaluation.

Determined eligible for Medical Assistance (MA) in Pennsylvania

Meet the financial requirements as determined by your local County Assistance Office

Individual cost limit of $85,000 per person per fiscal year (Supports Coordination is excluded from this limit)

Services we offer

In-Home and Community Support

Personal care is available for any daily tasks you may need support with, such as bathing, showering, applying creams and lotions, dressing and toileting, all in the comfort of your own home.

Companion

Companionship care is available and offers company and friendship. Companions may help with certain household chores; however, their primary purpose is to spend time with you or your loved one.

Respite

Respite care is available to provide temporary relief for a primary caregiver, enabling you to take a much-needed break from the demands of caregiving a sick, aging, or disabled family member.

Homemaker/Chore Service

This service provides assistance with household tasks and chores for individuals who may have difficulty managing these activities independently. This service includes: Household Cleaning and Maintenance: Ensuring the home remains clean and safe. Homemaker Activities: Meal preparation, laundry, and other essential tasks. This service is limited to 40 hours per year and is available when the individual or anyone else in the household is temporarily incapable of performing these tasks.

Community Participation Supports (CPS)

Our CPS service aims to support individuals with disabilities by promoting integration and inclusion within their communities. Key areas of focus include: Socialization: Developing social skills, understanding appropriate boundaries, and managing social discomfort. Employment: Providing vocational support, developing career goals, and promoting competitive employment and volunteer opportunities in alignment with Pennsylvania’s "employment first" initiative. Daily Living Skills: Teaching skills necessary for independent living, such as cooking, housework, managing finances, and maintaining personal hygiene.
